WELCOME

We are NYS DOH approved Service Coordination company for clients who have active Medicaid. The clients must be at least 18 Year of age or older. Need help with integration back into their home community and be from Traumatic Brain Injury (TBI) or Nursing Home Transition Diversion (NHTD) populations.

YHP Cares is a home health care agency, which in contract with the New York State Department of Health, provides services to seniors, survivors of traumatic brain injury and those wishing to maintain their community living by avoiding nursing home placement.
